If the cat has not been taken care of you need to get it to a vet ASAP. If the knots are very big, don't try to comb them out. Have a groomer or a vet cut them (don't do this yourself). You might also think about giving your cat a lion cut and just starting all over fresh. Most cats love the way this feels, especially in the hot summer. Your vet can also talk to you about putting your cat on an appropriate type of food and an eating schedule to help her lose some weight, as obesity in cats can lead to diabetes, liver, kidney and heart failure.
